Anxiety test (GAD-7)

The GAD-7 is a widely used 7-question screening test for anxiety. It takes about 2 minutes. Your answers about the last 2 weeks give a score from 0 to 21. Scores of 5, 10 and 15 mark mild, moderate and severe anxiety, and 10 or more calls for further evaluation. The result is not a diagnosis.

About this questionnaire

  • The GAD-7 (Generalized Anxiety Disorder-7) was developed by Drs. Spitzer, Kroenke, Williams and Löwe. Doctors use it widely to screen adults for anxiety.
  • The questions, answers and scoring on this page are the official text, without changes. The levels follow the cut-offs in the official manual. The manual names only the cut-offs of 5, 10 and 15; the name of the 0 to 4 range was written by this website.
  • The questionnaire is a screening tool, not a diagnosis. Only a doctor can diagnose an anxiety disorder, after a full assessment.

Scoring

  • Each question scores 0 (Not at all), 1 (Several days), 2 (More than half the days) or 3 (Nearly every day). The total is from 0 to 21.
