About this questionnaire
- The GAD-7 (Generalized Anxiety Disorder-7) was developed by Drs. Spitzer, Kroenke, Williams and Löwe. Doctors use it widely to screen adults for anxiety.
- The questions, answers and scoring on this page are the official text, without changes. The levels follow the cut-offs in the official manual. The manual names only the cut-offs of 5, 10 and 15; the name of the 0 to 4 range was written by this website.
- The questionnaire is a screening tool, not a diagnosis. Only a doctor can diagnose an anxiety disorder, after a full assessment.
Scoring
- Each question scores 0 (Not at all), 1 (Several days), 2 (More than half the days) or 3 (Nearly every day). The total is from 0 to 21.
| Score | Result | Meaning |
|---|---|---|
| 0–4 | Below the mild range | Your score is below the cut-off for mild anxiety (5). |
| 5–9 | Mild | You have mild symptoms of anxiety. This is below 10, the score at which the official manual recommends further evaluation. |
| 10–14 | Moderate | You have moderate symptoms of anxiety. A score of 10 or more calls for further evaluation. |
| 15–21 | Severe | You have severe symptoms of anxiety. A score of 10 or more calls for further evaluation. |
