About this questionnaire
- The following questionnaire can be used as a starting point to help you recognize the signs/symptoms of Adult ADHD but is not meant to replace consultation with a trained healthcare professional. An accurate diagnosis can only be made through a clinical evaluation.
- This Adult Self-Report Scale-V1.1 (ASRS-V1.1) Screener is intended for people aged 18 years or older.
- The 6-question Adult Self-Report Scale-Version1.1 (ASRS-V1.1) Screener is a subset of the 18-question Adult ADHD Self-Report Scale-Version1.1 (Adult ASRS-V1.1) Symptom Checklist.
- The questions, answers and scoring on this page are those of the official form, without changes. As on the original form, answers at the screening level have a darker background. For the original form and the latest guidance, please refer to the Harvard NCS website.
Scoring
- Questions 1 to 3 meet the screening level at Sometimes or more often; questions 4 to 6 at Often or more often. Each counts as 1, so the total is from 0 to 6.
| Score | Result | Meaning |
|---|---|---|
| 0–3 | Below the screening threshold | Fewer than 4 of your answers meet the screening level. This is below the threshold of this questionnaire. |
| 4–6 | Screened positive | Four (4) or more checkmarks indicate that your symptoms may be consistent with Adult ADHD. It may be beneficial for you to talk with your healthcare provider about an evaluation. |
